Post CRAFT Study Survey

To help us improve our program to best meet your needs the University of Utah’s College of Social Work Social Research Institute is studying the effectiveness of CRAFT Connect. Please complete this survey after your last Family Support Group session. Entering your name, email address and hitting “Send” downloads the completed survey and sends it to you within the hour. Having a record of your thoughts will increase your understanding and help you learn. As you progress through CRAFT Connect you will be able to measure that progress.

FEELINGS. The quality of your own life has probably suffered from the stress of dealing with your loved one’s mental health or substance use disorders. As a result, you may be watching them with widely varying emotions. At one extreme you may feel determined to help you loved one succeed and are convinced that they will. At the other extreme you may feel depressed, anxious or even skeptical. Having both positive and negative feelings about your loved one’s recovery is a natural and normal part of change.
